Clogged fuel filter— Most likely
A restricted fuel filter is the most common cause of low rail pressure. Replace the primary and secondary fuel filters before doing anything else.

Failing high-pressure fuel pump (HPFP)— Possible
The HPFP wears internally and loses the ability to build rail pressure, especially at low RPM. A fuel system pressure test will confirm.

Leaking fuel pressure regulator— Possible
The regulator on the fuel rail can leak internally, allowing pressure to bleed off. Easier and cheaper to check before condemning the pump.

Air leak in the low-pressure fuel supply— Less common
Air in the supply line prevents the HPFP from building full pressure. Inspect all fittings and the fuel tank pickup.
